Get your Passport Forms, Passport Photos, and Passport Questions Answered at Elizabethtown Public Library!
  1. Make an appointment by calling Elizabethtown Public Library at (717) 367-7467. Each person applying for a passport must be present when applying. Children 15 and under need both parents to be present. Teens between 16-18 must have proof of parental consent or a parent present at the time of application.
  2. Please note that passport processing times vary. Because of the number of passports in process, please allow AT LEAST 6-8 weeks for regular processing. You may expedite your application for an additional $60 per application plus 2 way Express Mail fees.
  3. Gather the following items to bring with you to your application appointment:

Proof of Citizenship

Birth Certificate -
This must be the original, state issued document with a raised seal. It should say "Birth Certificate" or "Certification of Birth" and be issued from the "Department of Vital Statistics." This will be sent with the passport application and returned with your newly issued passport.
         OR
Naturalization Certificate - This must be the original document with a raised seal.   This will be sent with the passport application and returned with your newly issued passport. 
        
Valid Photo Identification with Signature
 
Your driver's license is the best choice. If you are from out of state you must have a second form of identification. Picture ID is preferred, and the more information on your identification, the better.
 
Passport Photos
 
2 identical photos with very specific specifications must be submitted. (Please refer to U.S. Department of State Website.) You can get your passport photos taken at Elizabethtown Public Library for just $10 per person.
 
Checks
 
Personal checks are the best method of payment when applying for your passport. Two checks will be necessary: one to the U.S. Department of State and one to the Elizabethtown Public Library.
 
Here is an itemized list of the fees for passport books. (Amounts will vary depending on your situation, so please wait until your appointment to make out checks.)
    • Adults (16 and over):
      • $110 Passport Fee payable to U.S. Department of State
        • For expedite, add $60 (expedite fee), $18.30 (outbound Express Mail fee) and $14.96 (return Express Mail fee).
      • $25 Facility Fee payable to Elizabethtown Public Library
      • $10 for each set of photos
    • Children (15 and under)
      • $80 Passport Fee payable to U.S. Department of State
        • For expedite, add $60 (expedite fee) , $18.30 (outbound Express Mail fee) and $14.96 (return Express Mail fee).
      • $25 Facility Fee payable to Elizabethtown Public Library
      • $10 for each set of photos

Cash or MasterCard/Visa are accepted for the Facility Fee and Photos. These methods are also acceptable for the passport fee, but additional processing time is necessary.
 
You may purchase both a passport book and card at the same time if you want to do so. You would pay the passport fee for both the book and card, but only one facility fee.

Do I need a passport to go to Canada or Mexico?
It Could Be Yes OR No. (Visit
www.travel.state.gov for up-to-date info. )

Yes - ALL PERSONS traveling by air outside of the United States are required to present a passport or other valid travel document to enter or re-enter the United States.

No - If traveling by land or sea, U.S. citizens currently may present a passport OR , a driver’s license (or other government-issued photo ID) along with a birth certificate (or other proof of citizenship).
 
(Please Note: This requirement will change in 2009 so that everyone will need a US passport to travel outside of the United States, regardless of travel method.)

What is a passport card?
Passport cards are new, limited-use, wallet-size cards. Passport cards will only be valid for land and sea travel between the U.S. and Canada, Mexico, the Caribbean region, and Bermuda. They are less expensive than passport books, but valid for the same length of time. U.S. citizens may apply now for passport cards. They will be mailed as soon as they are available (Visit
www.travel.state.gov for up-to-date info.)

How do I get a passport at Elizabethtown Public Library?
Elizabethtown Public Library is a passport acceptance facility, receiving your application on behalf of the US Department of State. Please call (717) 367-7467 to make an appointment and make sure that you have everything and everyone needed to complete your application. Some things vary according to your circumstances.

At your appointment, you will have your picture taken (if not supplied), then ID and citizenship documentation examined. You will swear or affirm your application information is true and correct, then you will sign your application and ID will be documented. We will do our best to answer your questions. Elizabethtown Public Library will then send your completed application to a designated address given by the US Department of State. You should have your passport book in 4-6 weeks or less.
Passport cards cannot be expedited.
